All Fortify360 insurance products and services are offered only in Texas. Insurance activity is conducted only where Eric and Fortify360 have the required authority, appointment, certification, and product access.

Independent agency and comparison scope

Fortify360 is an independent insurance agency and is not limited to the products of one insurance company. Eric may compare suitable options available through the carriers and providers Fortify360 is authorized to represent.

Fortify360 does not represent every insurer, provider, product, policy, or plan in Texas. A comparison is limited to the options available through Fortify360 for the applicable product, location, eligibility, underwriting profile, and time.

The fact that a carrier or provider is named or displayed does not mean every product from that organization is available through Fortify360 or to every applicant.

Actual coverage is governed by the applicable policy, certificate, evidence of coverage, plan documents, endorsements, exclusions, limitations, eligibility rules, underwriting decision, and carrier or provider confirmation. Quotes, applications, and coverage changes

A quote is an estimate based on the information available at the time and is not a promise that coverage will be offered or issued. Rates, premiums, benefits, limits, deductibles, eligibility, and underwriting decisions may change after additional information or carrier review.

Products, carriers, providers, rates, benefits, networks, formularies, underwriting, eligibility, and service availability can vary within Texas and can change over time.

Fortify360 may add or remove carrier and provider relationships. Verify current availability with Eric and review the applicable carrier or provider materials before making a decision.

Compensation

Fortify360 may receive compensation from an insurance carrier, provider, or authorized distribution partner when a client purchases or enrolls in a product through the agency. Compensation arrangements vary by product and relationship.

Any client-paid fee, if applicable, must be disclosed before the related service is provided. Contact Eric if you have questions about how Fortify360 may be compensated in connection with a particular product.

Personal and business insurance

Descriptions of homeowners, auto, landlord, condo, renters, classic auto, umbrella, general liability, business-owner, commercial-auto, group-health, or other coverage are simplified summaries. Coverage names, definitions, limits, deductibles, exclusions, conditions, endorsements, underwriting, and availability differ by carrier and policy.

No website description should be used to determine whether a specific loss, business activity, contract, vehicle, property, person, or event is covered.

Life insurance, long-term care, IUL, and annuities

Life insurance, long-term-care insurance, indexed universal life insurance, and annuities involve eligibility, underwriting, costs, limitations, and product-specific risks. Not every product is appropriate for every person.

Insurance and annuity guarantees are subject to the terms of the contract and the claims-paying ability of the issuing insurer. Indexed products are not a direct investment in a stock-market index. Index crediting is subject to the contract's methods, caps, participation rates, spreads, floors, and other terms, which may change when permitted by the contract.

Policy loans, withdrawals, distributions, surrender, insufficient funding, or lapse may reduce values and benefits and may create tax consequences. Annuities may include surrender periods, withdrawal charges, market-value adjustments, tax penalties, or limitations on access to funds.

Health insurance

Medicare

Fortify360 Insurance Group is not affiliated with, endorsed by, or connected to the U.S. government or the federal Medicare program.

Fortify360 does not offer every Medicare plan available in every Texas service area. Information from Fortify360 is limited to the plans and products the agency is authorized to offer. Plan availability, premiums, benefits, provider networks, formularies, pharmacies, eligibility, and enrollment rules vary by county, ZIP code, carrier, and plan year.

Claims and emergencies

Website forms and ordinary messages do not file a claim. Follow the claims instructions in your policy documents or contact the carrier using its official claims channel.
